Every patient must be referred to our practice by a physician outside of our office. This includes, but is not limited to, primary care physicians, surgeons, or another specialty physician. We place a strong emphasis upon communicating with the referring physician, and will work with both the patient and other caregivers to provide the best consultative services possible.
We encourage you to have all of your prescriptions filled through your primary physician. Patients should have been seen in our office within a year in order to have prescriptions filled by our office. We ask that the courtesy of 48 hours notice be given for all prescription refills. We do not routinely refill prescriptions on weekends or after hours. It is our policy to avoid the prescription of narcotic analgesics (pain medications) during the weekend and after hours, during such times that the physician may not be your own physician.

We will gladly send in claims for other insurances, as long as discounted services have not been provided. Please remember that you are responsible for any deductibles, coinsurance or copays.
Payment is due for services at the time of each visit. If you are experiencing financial difficulties, we are willing to make special arrangements for periodic partial payments. Discounted fees are also available to patients with no insurance or for services not covered by your insurance. In these cases, we will not submit an insurance claim. If patients would like a claim submitted, a discount is unavailable. Our billing staff or practice administrator will be glad to discuss any fees, either before or after care is rendered. Accounts are handled by our billing staff on an alphabetical basis, so please contact the representative associated with your account by phoning 877-273-2943 and following the prompts.
